Instructions
There are three ways for you to fill out the Robes Survey:
1. QR Code
2. Fill out online
3. Fill out by hand and send by email or postal mail

(on this downloadable copy you can also scan the QR Code on the Survey with your mobile device as follows:
- - Hold your camera up to the QR Code (be sure QR code is within the frame)
  - A banner will appear in your camera view. Touch that banner and you will be taken to the survey which you can fill out on your mobile phone

Note:
All responses (paper or electronic) must be submitted by individual members.
The Survey is due by January 31, 2024.
